Who we are

Yolanda's Medical Pharmacy is an independent, owner-operated community pharmacy located at 2536 County Road 27, Woodslee, Ontario. This Privacy Policy explains how we handle personal information submitted through this website (yolandasmedicalpharmacy.ca).

Yolanda Stanczak, RPh, is the Health Information Custodian responsible for the information collected here.

What we collect on this website

Our website forms (“New Patient” callback, “Refill Callback”, “Newsletter”, and contact requests) collect only the contact information needed to reach you back:

• Your name
• Phone number
• Email address (optional)
• The name of your previous pharmacy, if you are transferring
• Your delivery / pickup preference
• Any free-text notes you provide

We do not collect your OHIP / health card number, date of birth, allergies, current medications, insurance details, or other clinical information on this website. We collect that information verbally over the phone when we call you back, or in person at the pharmacy counter, in accordance with the Personal Health Information Protection Act (PHIPA) and the Ontario College of Pharmacists’ record-keeping standards.

How we use it

We use the contact information you submit to follow up with you about your request — to call you back to set up your patient record, arrange a prescription transfer, confirm a refill, or answer your question. We do not sell, rent, or share your information with third parties for marketing purposes.

How long we keep it

Website callback requests are kept until follow-up is complete, then archived for our internal records. Any clinical information collected during follow-up becomes part of your pharmacy record, which is retained for at least ten years from the last professional pharmacy service provided to you, as required by Ontario Regulation 264/16.

Where it is stored

Information submitted through this website is stored in a database hosted in Canada (AWS Canada Central region). Access is restricted to authorized pharmacy staff and is encrypted in transit and at rest.

Your rights

You have the right to access the information we hold about you, ask us to correct it, or ask us to delete the information we've collected through this website. Clinical pharmacy records are subject to the retention requirements above. To make a request, contact us using the details below.
